Creatine spent three decades filed under gym supplement: a plastic tub in a locker room, mentally shelved next to heavy barbells and protein shakes. Lately it turns up in very different conversations — whether it helps older adults hold on to muscle, whether it does anything for memory or mental fatigue, whether it belongs in a routine that involves no lifting at all. Some of that interest rests on genuinely interesting research. Some of it is enthusiasm running ahead of the evidence. Separating the two starts with what creatine actually is.
What Creatine Actually Is
Creatine is a compound the body already makes, assembled in the liver and kidneys from amino acids, and it also arrives in the diet through red meat and fish in fairly small amounts. Most of what is in the body sits in skeletal muscle, much of it stored as phosphocreatine. Its job is unglamorous and specific: helping regenerate the cell’s immediate energy currency during very short, very hard efforts — the first several seconds of a sprint, a heavy set, a flight of stairs taken quickly. Muscle stores are not usually full, and supplementation raises them modestly, which is the whole mechanism. That narrow, well-understood job is why creatine became one of the most heavily studied supplements in sports science.
Why the Interest Moved Past the Weight Room
Two threads widened the audience. The first is aging. Muscle mass and strength decline with the years, and that decline is tied to independence, balance and fall risk, so researchers have looked at whether creatine alongside resistance training helps older adults preserve more of both. The second is the brain, which also uses phosphocreatine and is energy-hungry in a similar way. That prompted studies on whether supplementation affects mental performance under stress, sleep deprivation, or in people whose baseline stores are lower, such as those eating little or no meat. Those studies are mostly small and short, results are mixed, and the effects reported are modest rather than dramatic. Interesting is the right word for the cognition research. Established is not.
What the Evidence Supports So Far
Read across the literature and a reasonably clear picture emerges of where confidence is high and where it is thin.
- Strength and power output, when paired with training. This is the best-supported effect by a wide margin, and it shows up mainly in repeated short, intense efforts rather than endurance work.
- Muscle mass alongside resistance training in older adults. Several trials point this direction, but the training is doing the heavy lifting; creatine is not a substitute for it.
- Cognitive effects under stress or sleep loss. Preliminary and inconsistent. Early findings are worth watching, not worth building expectations on.
- Not a fat burner and not a protein replacement. It does not change body composition on its own and it does not cover a diet short on protein.
- Some early weight gain is water. Creatine draws water into muscle cells, so the first pounds on the scale usually reflect that rather than tissue.
Safety, Side Effects, and Sensible Caution
Creatine has a long research record in healthy adults, and the concerns that circulated for years, particularly about kidney damage, have not generally been borne out in that population. The complaints that do show up are ordinary: stomach upset, some bloating, occasional cramping, most often when a large amount is taken at once rather than spread out. The research protocols that found benefits generally used a small daily amount taken consistently over weeks, not heroic doses.
Caution belongs where the research thins out. Anyone with kidney or liver conditions, anyone taking regular medication, anyone pregnant or nursing, and anyone under eighteen sits outside most of the study populations, and that is a conversation to have with a doctor or pharmacist before starting, not after. Supplements are also loosely regulated, so a third-party testing seal on the container is worth more than any claim printed on the front. And if the goal is stronger legs at seventy or a clearer head at forty, sleep, protein, and actual resistance training remain the parts of the picture with the most evidence behind them.
